Sciatica vs. General Low Back Pain: How to Tell What’s Actually Wrong

The simplest way to tell sciatica from general low back pain is to notice where the pain travels. Low back pain usually stays in the lower back and feels like an ache, stiffness, or soreness. Sciatica involves a nerve, so the pain shoots from the lower back down through the buttock and leg, often with tingling or numbness. At HealthFirst Chiropractic, our doctors use a careful exam to find which one you have, because the right diagnosis points to the right treatment.

What Is General Low Back Pain?

Low back pain is one of the most common reasons people see a chiropractor. It typically stays centered in the lower spine and the muscles around it.

The pain might feel like a dull ache, a tight band of stiffness, or a sharp catch when you bend or twist. It often comes from muscle strain, joint restriction, poor posture, or a misaligned segment in the lumbar spine.

This kind of pain is usually mechanical, meaning it relates to how the structures of your back are moving and loading. It tends to stay local rather than spreading down the leg.

What Is Sciatica?

Sciatica is not a condition on its own so much as a symptom of something pressing on or irritating the sciatic nerve. That nerve runs from your lower back, through the buttock, and down the back of each leg.

When something compresses it, you feel pain along its path. The discomfort can reach the thigh, the calf, or even the foot, often on just one side of the body.

Common causes include a herniated or bulging disc pressing on the nerve root, spinal stenosis, or joint dysfunction in the lower spine. Because a nerve is involved, the symptoms feel different from a simple muscle ache.

Key Differences in How They Feel

Telling the two apart is easier when you pay attention to a few specific signals. Patients describe them in noticeably different ways.

Where the Pain Travels

Low back pain mostly stays put in the lower back. Sciatica radiates, following the nerve down into the buttock and leg.

If your pain stops at your waistline, it is more likely mechanical low back pain. If it travels below the knee, a nerve is usually involved.

The Type of Sensation

Low back pain tends to feel like aching, soreness, or stiffness. Sciatica often brings sharp, burning, or electric sensations, sometimes paired with tingling, pins and needles, or weakness in the leg.

That nerve-related quality is a strong clue. Muscles ache, but irritated nerves tend to burn or shoot.

Why the Right Diagnosis Matters

Treating sciatica as if it were ordinary muscle pain often falls short, because the real problem is what is pressing on the nerve. Until that source is addressed, the leg symptoms tend to return.

The reverse is also true. Aggressive nerve-focused treatment is unnecessary for a straightforward muscle strain, and it can add cost and time you do not need.

This is exactly why we do not adjust anyone on the first visit. Treating the wrong cause is easy to do when nobody confirms the source first.

How We Pinpoint the Source at HealthFirst

Our Gonstead approach is built for this kind of question. On your first visit, our doctors take a full history and perform orthopedic and neurological testing to see whether a nerve is involved.

Digital X-rays, taken on-site at both our Westerville and Pickerington offices, show the structure of your lumbar spine, including disc spaces and alignment. Hands-on palpation and motion testing fill in how each segment is moving.

On your second visit, your doctor explains the result of tests and lays out a plan based on what is actually causing your pain. For disc-related sciatica, patients at our Pickerington office may also be candidates for spinal decompression, a non-surgical option that reduces pressure on the disc.

When to Get It Checked

Mild low back soreness sometimes settles with rest and gentle movement. Pain that radiates into the leg, lingers beyond a week or two, or comes with numbness or weakness deserves a proper evaluation.

The sooner you know which problem you are dealing with, the sooner the right care can begin. You can read more about how we approach low back pain and sciatica, or explore our full chiropractic care.
